IMPORTANT CANCELLATION NOTICE

Before you proceed with canceling your policy, please be aware of the serious consequences that may follow:

1. LOSS OF COVERAGE
   By canceling, you will forfeit all insurance coverage for dental, vision and hearing for you and your family.
2. LIMITED OR NO FUTURE ENROLLMENT OPPORTUNITIES
   If you cancel now, you may lose your chance to secure affordable coverage in the future. Reapplying could result in higher premiums, stricter terms, or even denial of coverage.
3. POTENTIAL LOSS OF GOVERNMENT SUBSIDIES
   If your policy is part of a program that qualifies for government subsidies, cancellation could result in the loss of these financial benefits. This could leave you without support in the event of an unexpected hardship.

Confirm Your Identity

To process a cancellation request, we require your Date of Birth and the last four digits of your Social Security Number due to the sensitive financial nature of these services. This ensures the security and accuracy of your request.
